12 Panel Drug Test: What It Detects & Cup Buyer's Guide

12 Panel Drug Test: What It Is and Who Should Use It

A 12 panel drug test is a urine screen that checks one sample for 12 drug classes at once, usually in an instant-read cup. Most configurations cover amphetamines, barbiturates, benzodiazepines, cocaine, MDMA, methamphetamine, methadone, opiates, oxycodone, PCP, THC, and one more analyte that varies by manufacturer. It suits clinics, treatment centers, courts, probation offices, and employers who need broader coverage than a 5- or 10-panel test.

American Screening Corp (ASC) supplies these tests wholesale from Shreveport, Louisiana. This guide covers what the panel detects, how the cup works, and what to confirm before you buy in volume.

What Does a 12 Panel Drug Test Test For?

There is no single standard 12-panel menu. Manufacturers choose their own drug list, so always read the package insert for the exact lot you are buying. A common configuration includes:

    • AMP – amphetamines
    • BAR – barbiturates
    • BZO – benzodiazepines
    • COC – cocaine metabolite (benzoylecgonine)
    • MDMA – ecstasy
    • MET – methamphetamine
    • MOP/OPI – morphine and other opiates
    • MTD – methadone
    • OXY – oxycodone
    • PCP – phencyclidine
    • THC – marijuana metabolite
    • TCA or PPX – tricyclic antidepressants or propoxyphene, depending on the maker

Some panels swap in other analytes, such as buprenorphine or fentanyl. If fentanyl, buprenorphine, or synthetic opioids matter to your program, confirm they are on the panel instead of assuming a 12-panel includes them.

Cutoff levels matter as much as the drug list

Each drug is reported against a cutoff concentration in ng/mL. Below the cutoff the result reads negative, and at or above it the result is non-negative (presumptive positive). Cutoffs differ between products, so compare them against your policy or program requirements before standardizing on one test.

12 Panel Drug Test Cup: How It Works

A 12 panel drug test cup combines collection and testing in one device. The donor provides a urine sample in the cup, and a temperature strip on the cup shows whether the sample is within the expected range. The lid or a built-in test card then exposes the sample to the test strips, and the results are read after a few minutes, typically within five minutes.

Each drug line is read independently. Most cups use a line-present-means-negative design, so a missing line at a drug position indicates a non-negative result for that drug. Follow the instructions for your specific product and train staff to read results the same way every time.

An instant result is a screening result only. A non-negative should be treated as presumptive until a laboratory confirms it with a method such as GC-MS or LC-MS/MS. ASC works with lab confirmation services for this step, so you can pair onsite screening with a confirmation path.

    Cup vs. dip card

Both formats use the same immunoassay approach. The difference is handling and workflow. A cup holds the specimen in a closed device, which reduces handling and spill risk. A dip card is a lower-profile option for programs that collect in a separate container. You can browse drug testing cups or urine drug test dip cards to compare formats.

12 Panel vs. 5 Panel vs. 10 Panel

Panel Typical coverage Best fit Watch out for
5 panel THC, cocaine, opiates, amphetamines, PCP Employers aligned with the federal drug list; baseline screening Misses benzodiazepines, barbiturates, oxycodone, and methadone
10 panel The 5 above plus drugs such as benzodiazepines, barbiturates, methadone, and others Clinics and employers wanting wider coverage The 10 drugs vary by maker
12 panel 10-panel coverage plus additional analytes such as MDMA, oxycodone, or TCA Treatment centers, courts, probation, hospitals, programs monitoring prescription misuse More drug lines mean more prescription-drug flags to review

Where a 12 Panel Fits, and Where It Does Not

    Good fits

    • Addiction treatment and clinics: broad coverage helps monitor patients on or off medication-assisted treatment.
    • Courts and probation: a single cup covers many substances in one visit.
    • Staffing firms and non-regulated employers: useful when your written policy covers more than the federal five.

Not a substitute for DOT testing

Under 49 CFR Part 40, DOT-regulated drug tests require specimen collection on the federal custody and control form and analysis at an HHS-certified laboratory. An instant 12-panel cup does not meet that requirement. For regulated workers, use ASC's DOT-compliant collection kits and a lab, and reserve the 12-panel cup for non-regulated programs.

    Prescription medications

Several panel drugs, such as benzodiazepines, opiates, oxycodone, amphetamines, and barbiturates, can be legally prescribed. A non-negative result does not show misuse. Build a review step into your process, such as a medical review officer for employers or a clinician for patient programs, and send specimens for lab confirmation before any adverse action.

    State law and policy

Rules on marijuana, notice, and adverse action vary by state and industry. Have counsel review your testing policy. For pre-employment screening, many employers pair drug testing with background screening through our sister division, Reveal Background.

    Buyer Checklist Before You Order

    • Get the exact drug list and cutoff concentrations from the package insert, and confirm they match your policy.
    • Check that the device is FDA-cleared for the intended use and that the lot has current expiration dating.
    • Confirm whether the cup includes a temperature strip and whether you need adulterant checks.
    • Decide your confirmation path for non-negative results before the first test is run.
    • Standardize result-reading training and documentation across staff and sites.
    • Verify storage requirements and shelf life against your expected usage so stock does not expire.
    • Ask the supplier about reorder consistency, because switching manufacturers can change the panel or cutoffs.

    Frequently Asked Questions

What does a 12 panel drug test test for?

It screens urine for 12 drug classes. These typically include amphetamines, barbiturates, benzodiazepines, cocaine, MDMA, methamphetamine, methadone, opiates, oxycodone, PCP, THC, and one more analyte that varies by manufacturer. Check the package insert for the exact list.

What is a 12 panel drug test cup?

It is an all-in-one urine collection device with built-in test strips for 12 drugs. It usually includes a temperature strip, and results are read within minutes. Non-negative results are presumptive until a lab confirms them.

Is a 12 panel drug test accepted for DOT testing?

No. DOT-regulated tests must follow 49 CFR Part 40, which requires lab analysis under federal custody and control procedures. Use an instant cup only for non-regulated programs.

Can a 12 panel drug test give a false positive?

Yes. Immunoassay screens can react to certain medications or cross-reactive substances. That is why a non-negative result should be confirmed by a lab and reviewed in light of any valid prescriptions.
