What is a 12 panel drug test?
A 12 panel drug test screens for twelve substances in a single urine cup, extending coverage well beyond the SAMHSA-5 to include prescription opioids, designer drugs, and commonly-abused depressants. The exact configuration varies by manufacturer, but ASC's standard 12 panel covers:
- Amphetamines (AMP)
- Barbiturates (BAR)
- Benzodiazepines (BZO)
- Buprenorphine (BUP)
- Cocaine (COC)
- Marijuana (THC)
- Methadone (MTD)
- Methamphetamine (MET)
- MDMA / Ecstasy
- Opiates (OPI)
- Oxycodone (OXY)
- Phencyclidine (PCP)
Higher-panel cups (14 and 16 panel) add fentanyl, tramadol, propoxyphene, and additional designer drugs depending on use case. For DOT-regulated testing, the 5 panel SAMHSA configuration is required — see our DOT drug testing supplies.
Who uses 12 panel drug tests?
- Non-DOT employers in high-risk industries (construction, manufacturing, healthcare, mining)
- Addiction treatment programs requiring expanded substance coverage
- Probation, parole, and drug courts testing for prescription medication misuse
- Hospitals and emergency rooms for clinical toxicology screening
- Pain management clinics for medication compliance monitoring
Why 12 panel instead of 5 panel?
The 5 panel SAMHSA configuration was developed in the 1980s. The substance landscape has since shifted significantly toward prescription opioid misuse (oxycodone, hydrocodone), benzodiazepine dependence, and synthetic substances. A 12 panel test captures these additional risks that a 5 panel misses entirely.

12 panel drug test FAQ
Is a 12 panel test more accurate than a 5 panel?
No — accuracy for each substance is comparable (typically 97-99% vs. laboratory GC-MS confirmation). A 12 panel test isn't more accurate, it screens for more substances. The additional panels detect prescription opioids, benzodiazepines, and synthetic drugs that a 5 panel doesn't cover.
Can I use a 12 panel test for DOT compliance?
No — DOT-regulated testing under 49 CFR Part 40 requires the specific SAMHSA-5 panel configuration. Using a 12 panel test for DOT-regulated employees is a compliance violation. Non-DOT testing can use any panel configuration. See DOT drug testing supplies.
What does the 12 panel test detect for fentanyl?
ASC's standard 12 panel does NOT include fentanyl. Fentanyl screening requires a 13+ panel or a separate fentanyl-specific test. Contact our team about fentanyl-inclusive cup configurations.
